Acknowledgment of Risk and Release of Liability

I, the undersigned participant, acknowledge that participation in the Total Balance in Motion 5K Run/Walk & Fun Run (the “Event”) involves physical activity that may carry inherent risks, including but not limited to bodily injury, illness, or even death. I certify that I am physically fit and have not been advised of my inability to participate in physical activity by a medical professional. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in the Event. I voluntarily assume full responsibility for all risks associated with my participation in the Event, including but not limited to accidents, falls, contact with other participants, weather conditions, and negligence of the Event organizers, sponsors, volunteers, or any other parties involved. In consideration for being permitted to participate in the Event, I hereby waive, release, discharge, and covenant not to sue Total Balance Therapy, the Event organizers, sponsors, volunteers, officials, and any other associated entities (collectively, the “Released Parties”) from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by me while participating in the Event. I agree to abide by all Event rules, instructions, and decisions of Event officials. I grant permission to the Released Parties to use any photographs, videos, or recordings of me taken during the Event for promotional, marketing, or educational purposes without compensation. By signing below, I acknowledge that I have read, understood, and voluntarily agree to this Waiver and Release of Liability.
